Disability Case Management Programme
When life becomes complex, coordinated care makes all the difference. Our Disability Case Management Programme adopts a client- and family-centred approach, providing holistic support and specialised interventions that bring together every aspect of your loved one’s journey—healthcare, behavioural, emotional, and social.
Our services include:
- Intensive case management over 9 to 12 months by disability-trained social workers
- Specialist support and tailored interventions by a multidisciplinary team trained in disability and complex needs
- Personalised care plans that address the immediate and evolving needs of the individual
- Family-centric support including practical coaching for caregivers
- Transition planning and setup of future care arrangements
This programme is especially helpful for persons with IDD with complex or high support needs, offering families comprehensive, coordinated support, and peace of mind every step of the way.
Community Forensic Services
For persons with IDD, navigating the justice system can be complex and overwhelming. MINDS provides support across every stage—from arrest to rehabilitation and reintegration—ensuring their rights, dignity and well-being are upheld throughout the process.
Our approach includes:
- Advocacy and emotional support during arrest, investigation and legal proceedings
- Customised rehabilitation plans for offenders with disability
- Ongoing behavioural and social support throughout sentencing and reintegration
- Facilitating communication between families, legal authorities and community partners
- Support for victims and witnesses with disabilities affected by crime

Appropriate Adult Scheme
No one should face a police interview alone, especially individuals with IDD who may find communication or understanding their legal rights challenging. The Appropriate Adult (AA) Scheme provides vital support and protection for persons with disabilities, autism spectrum disorder or mental health conditions during interviews with law enforcement.
What you can expect:
- Trained volunteers who act as trusted intermediaries, safeguarding rights and facilitating communication
- Compassionate support that offers credibility and peace of mind in vulnerable moments

Home-Based Care
When travelling to a MINDS Hub isn’t possible, MINDS brings professional care directly to your home, delivering support where it is most comfortable and familiar.
Our home care services are provided by a multi-disciplinary team of Allied Health and care professionals who offer:
- Personalised care tailored to individual needs, building essential life skills
- Therapy and social support that improve physical and emotional well-being
- Professional caregiver training to boost confidence and develop long-term care plans
- Flexible scheduling that respects your family’s pace and respite needs
- Consistent, familiar support that fosters strong relationships
Ideal for individuals with mobility challenges or high support needs, our home care services are a vital source of comfort and assistance for families of persons with IDD.
Ageing Programme
MINDS supports adults aged 45 and above in the IDD community who may be experiencing a decline in daily living skills, physical abilities or work interests. At the heart of our support is our Oaktree Programme. Starting with Oaktree@Woodlands, the dedicated centre offers holistic programmes that promote health, well-being, connection, and social engagement.
Through the Oaktree Programme, your loved one will benefit from:
- Health Management: Structured exercise, therapy and preventive screenings to support functional health.
- Social & Cognitive Engagement: Weekly games, interest-led workshops and recreational outings that foster connection, joy and active ageing.
- Care Planning & Coordination: Collaborative goal-setting with families; ongoing check-ins, conferences and future planning to ensure care evolves with changing needs.
Programme executives, therapists from the Allied Health Professionals Hub, volunteers, and community partners work seamlessly together to provide synchronised care. Your loved one will receive social and daily stimulation and a strong support network, while you receive practical training, resources and guidance to build your confidence and care skills.
With structured programmes, regular health check-ups and continuous support, persons with IDD can age with dignity, surrounded by respect, care and love.
Clinic Services

- Occupational therapy to develop motor, cognitive, social, and adaptive skills
