Admission and Referral
Helping your loved one access the right support starts with understanding what they need and how to get there. MINDS’ admission and referral process is designed to simplify this journey, providing thoughtful guidance every step of the way, rooted in a person-centred, family-oriented approach.

Where Your Journey Begins
MINDS welcomes persons with intellectual disability (ID) and their caregivers at every stage. We are committed to supporting your loved ones in finding the right services to meet their needs.
We conduct a detailed consultation lasting approximately 1.5 to 2 hours, during which we explore health concerns using a structured questionnaire. A clinical examination will be performed, including measurements of height, weight, body mass index (BMI), and blood pressure. Please bring along any relevant medical records, medications, referrals, and health booklets for the consultation.
If your loved one has a disability with complex needs—such as diagnosed or suspected ID and/or autism with high behavioural needs or communication challenges—you may apply for the MINDS DCMP for enhanced support.
We currently serve residents living in the East, North-East, Bishan, Geylang, Marine Parade, or Toa Payoh regions. If you are living outside of these regions, you will be referred to Rainbow Centre. For clarifications or any other enquiries, please contact us.
- Other Eligibility Criteria:
- Your loved one must be a Singapore Citizen, or Permanent Resident with at least 1 immediate family member who is a Singapore Citizen.
- Your loved one is aged 60 and below.
- You or the primary caregiver must be committed to working with MINDS Staff.
- Process: Complete the form here and receive an acknowledgement from us within 1 to 2 working days.
Families can apply for special education schools through the Ministry of Education (MOE).
- Eligibility: Singapore Citizens or Permanent Residents entering Primary 1 or Junior 1, with priority given to Singapore Citizens. Returning Singaporeans may apply for all levels.
- Process: Submit an e-application to MOE, which will refer your application to the most suitable school based on your child’s profile. The school will then arrange interviews, assess your child’s suitability and confirm admission if a vacancy is available.

You may apply for a place in our SSCC if your child is between 7 and 18 years old and currently enrolled in a Special Education School in Singapore.
- Other Eligibility Criteria:
- Your child must be a Singapore citizen. For Singapore Permanent Residents (SPR), at least one immediate family member (parent or sibling) must be a Singapore citizen.
- The application must be supported by a diagnostic report or written statement from a relevant medical professional or psychologist.
- Your child must have suitable transport arrangements to and from the SSCC.
- Your child should be able to work in small groups and require minimal to moderate assistance with activities of daily living.
- Children who require one-to-one supervision or those with tendencies towards self-harm or violent behaviour; children with mental health issues, severe chronic medical conditions, frequent seizures, heart illness, or tube feeding needs will not be considered for enrolment.

The journey typically flows like this:
- Enquiry by the person with ID or their caregiver
- Assessment conducted by MINDS (if required)
- Service matching based on individual’s preferences, interests, needs, and strengths
- Referral outcome: your loved one is either onboarded into MINDS or referred to a more suitable programme or social service agency
Support Throughout the Process
We are with you every step of the way. Your assigned case manager or designated point of contact will assist with documentation, scheduling assessments and answering any questions you may have. We strive to make communication smooth and accessible—please let us know if you need language support or translation services.
