More than just a Health Booklet

Our personalised health passport records more than just medical information. We detail your child/ward’s communication skills, mobility needs, and behaviours of concern, enabling medical professionals and specialists to provide a more efficient and accurate assessment of health conditions.

Our healthcare professionals take every effort to ensure your child/ward is comfortable during the consultation. Our personal approach means we spend more time with you during each consultation as we walk caregivers through various options, adopting a lifespan approach to meet the healthcare needs of every individual with special needs.

Providing Referrals and Recommendations

We provide referrals and recommendations to appropriate healthcare specialists and agencies, enabling you to experience a simple and more understandable path while navigating through the complex healthcare system.

  • Complimentary Health Passport that details your child/ward’s medical summary, health action plan and related information.

What to Expect During the Consultation

  • To understand your child/ward with special needs better, we do a detailed consultation that takes about 1.5 – 2 hours, delving into health concerns using a structured questionnaire.
  • A clinical examination where height, weight, body mass index (BMI), blood pressure, and a systemic examination will be conducted. Do bring along your child/ward’s medical records, medications, and health booklet for the consultation.
  • Vision screening and provision of spectacles will be conducted as necessary.
  • Appropriate healthcare referrals will be given where necessary.
  • Resources linking to appropriate services and agencies will be provided where necessary.
