Disability Case Management Programme

Disability Case Management Programme (DCMP) is a holistic case management service that provides specialised interventions to persons with intellectual and/or developmental disabilities with complex or high-support needs. 

DCMP offers: 

  • Intensive case management by social workers
  • Multi-disciplinary and disability-specialised interventions and support
  • Family-centric interventions including emotional support, practical coaching and psychoeducation for families and caregivers 
  • To bridge persons with disabilities and their families to suitable disability programmes, social services or community resources
  • To put in place care transition plans and arrange for the future care of persons with disabilities whose caregiving arrangements are at risk of breaking down

Community Forensic Services

Community Forensic Services adopts a community-based case management approach in supporting offenders, victims and/or witnesses with ID through the stages of arrest, investigation, prosecution, trial, sentencing and post-sentencing.

This service is pertinent in the rehabilitation of offenders with ID and addressing their offending behaviour, as well as in the assistance of victims and witnesses with ID affected by crime. The intensive case management framework for CFS comprises of three-pronged interventions of working with the client, working with the family and providing linkages to community resources.

CFS will establish new capability in the area of forensic services for MINDS, which currently no other ID SSAs have ventured into. CFS will serve to plug the service gap in the social service and criminal justice systems by serving the needs of offenders, victims and witnesses with ID. Through this pilot service, MINDS can have the advantage of positioning ourselves as an industry leader in this area of forensic work with PWIDs.
